Location:ca 5 km W of Karachi-Jinnah International Airport (KHI) (   Pakistan)
Phase:Initial climb (ICL)
Nature:Cargo
Departure airport:Karachi-Jinnah International Airport (KHI/OPKC), Pakistan
Destination airport:Khartoum-Civil Airport (KRT/HSSS), Sudan
Flightnumber:4412
Narrative:
An Ilyushin 76TD cargo plane, registered 4L-GNI, was destroyed in an accident near Karachi-Jinnah International Airport in Pakistan. All eight occupants and three persons on the ground were killed.
The airplane operated on a cargo flight from Karachi-Jinnah International Airport (KHI), Pakistan to Douala, Cameroon with a planned en route stop at Khartoum, Sudan. The flight was loaded with 30.5 tons of relief supplies (tents).
After completion of necessary pre flight requisites by the flight crew, the cockpit crew contacted Karachi Airport at 01:14 hours local time and informed about their readiness to start for Khartoum. After receiving permission from the air traffic controller, the crew started the engines between 01:21 and 01:24. On previous flights the crew had encountered problems starting engine no.4. On those occasions the engine was finally started after switcing on the Engine Anti Ice System. At Karachi, the flight crew proactively activated the Engine Anti Ice System before starting engine no.4.
The most probable reason of abnormal starts of the engine no.4 was reduction of compressor efficiency because of blade wear. The service wear of blades increases axial and radial clearances and the blade surface becomes rough, thus reducing the compressor efficiency. With the inlet guide vanes heating mode on, the amount of air through the compressor’s first stages increases thus improving the start reliability.
The crew then taxied the plane to runway 25L and took off at 01:43 hours. Shortly after takeoff, as the aircraft was climbing through an altitude of 100 meters, the no.4 engine suffered an uncontained failure. The airspeed was 325 km/h and undercarriage had been retracted. The remaining three engines continued working on takeoff power.
The aircraft banked right, which the pilot tried to counteract with ailerons and rudder. The no.4 engine was then set to idle and subsequently shut down.
Despite fully deflected ailerons and spoilers to create a left bank, the aircraft continued in a right bank. Right bank had increased to 32° while the recorded deflection of the rudder was -24° (to the left) and elevator to -10° for pitch up. The aircraft reached a height 140 m before it entered a descent.
At 01:44 the bank angle became 71° to the right and the aircraft continued moving to right and impacted buildings that were under construction.

Probable Cause:

Cause of Occurrence:
The cause of the occurrence was uncontained failure of the 2nd stage disk of LP compressor of Engine # 4 due to fatigue fracture which resulted in in-flight fire and damage to adjacent areas of right wing / flaps to an extent that flight could not be sustained.
The use of mishap engine beyond its manufacturer’s assigned life without assessment and life enhancement by the manufacturer was the cause of its uncontained fatigue failure.

What is VATSIM?

VATSIM (short for the Virtual Air Traffic Simulation Network) is a completely free online platform which allows virtual pilots, wherever they are in the world, to connect their flight simulators into one shared virtual world. VATSIM also simulates air traffic control in this virtual world, creating the ultimate as-real-as-it-gets experience for you, the virtual aviation enthusiast.
With over 76,000 active members in 2014, connecting your flight simulator to VATSIM allows you to join hundreds of other pilots in our virtual skies, flying all kinds of aircraft all over the world. As a VATSIM member, you can join our many communities across the world to enhance your flying skills and flight simulator experience in a shared virtual airspace. You can also become a virtual air traffic controller, adding that extra bit of realism for you and your fellow enthusiasts. The real beauty of VATSIM is that it caters people who are completely new to the hobby as well as to the seasoned flight simmer.

The beginning

VATSIM was founded in July 2001 by a group of individuals who came together with the idea of creating an online community which truly met the needs of flight simulator enthusiasts across the world. As well as providing a common network for flight simulation enthusiasts to connect to, VATSIM aimed to create a global community for learning, sharing experiences and pursuing new opportunities in the virtual world. That original group of individuals are now referred to as the VATSIM Founders, and help to steer the direction of the network.

Our Structure

VATSIM is led by the Board of Governors, a group of individuals who each take on a specific specialism on the network to lead. The Board of Governors are supported by an Executive Committee, who represent the Regions of VATSIM, who are in turn supported by local staff structures. Everyone is a volunteer.
The VATSIM world is divided into Regions, then Divisions and finally local facilities, which take on a number of different names depending on where you are in the world. These structures help to build and maintain communities across the globe, providing resources, training and a place to share the experience of VATSIM wherever you are. VATSIM is also the main network for many established Virtual Airlines and other virtual flying groups, whose pilots populate our skies every day with hundreds of flights.
The most important part of the network is you: our members. You are free to enjoy VATSIM in the way you want to enjoy it, by flying, controlling, or both. VATSIM is a network for all to enjoy, and it can be whatever you make it. Welcome.

Event:
Aviation Awareness Expo
In an effort to Aviationize Pakistan, we need to ensure dissemination of relevant information regarding ‘Aviation & Aerospace’. Awareness and opportunities for employment in the aviation domain are important aspects for career seekers.

Career tracks’ information must be made readily available at appropriate forums. Similarly, information regarding educational opportunities to acquire relevant knowledge and skill-sets is essential for prospective candidates.

If education is provided and awareness is lacking the real potential will not be realized, and if only awareness is provided and education institutions are not provided the awareness exercise may be an effort in futility.

While efforts are underway for establishing Aviation & Aerospace specific educational institutions, hopefully nationwide in due course of time, a series of awareness programs in the form of road-shows, seminars, symposiums, radio and TV shows, etc. shall be planned and executed in a well-coordinated fashion. Aviation Awareness Expo is the first of the aviation awareness program being planned and executed, Insha’Allah.

The objective of the Aviation Awareness Expo is to make prospective candidates aware of the career tracks that the aviation industry has to offer.

Participants: Display stalls planned for …
• PAF (managed by PAF Recruitment & Selection Center)
• Individual stalls for PIA, Airblue, Shaheen, and possibly foreign airlines
• PCAA, SUPARCO

• PAC Kamra
• Aero Modelling Club (MASK)
• Aviation Support Organizations (SAPS, RAS)
• Training organizations
• Travel agencies (major ones only)

An accused can go abroad, says SC judge

An accused can go abroad, says SC judge

ISLAMABAD: A Supreme Court judge wondered on Wednesday what terrible harm might come to the state if model Ayyan Ali, caught on money-laundering charges, was allowed to go abroad.
“A person accused of committing some crime can go abroad under the law even for a pleasure trip,” observed Justice Ejaz Afzal Khan, adding that so long as she was not proven guilty she could go abroad and in her absence prosecution could record evidence.
The three-judge Supreme Court bench reserved its ruling on a set of appeals moved by the interior ministry as well as the Collector of Customs, Islamabad, against the Sindh High Court judgment of March 7, which called for removing the name of Ms Ali from the Exit Control List.
Ms Ali was caught on March 14, 2014, at the Benazir Bhutto International Airport, Islamabad, while trying to smuggle nearly half a million dollars out of the country.
Advocate Farhat Nawaz Lodhi, representing the customs collector, cited July 31, 2014, agreement signed between the model and her manager Barac Florinel-Eftimica who represents her worldwide interests regarding her work as artist and model and argued that if she was allowed to leave for Dubai her engagements may require her to stay in the UAE for two years. “We have to weigh things to determine whether she will come back or not,” the counsel emphasised.
The bona fide of the accused could be gauged from the fact that instead of pursuing the pending case before the special court by requesting for early hearing, she applied for the release of passport and then fought for removal of her name from the ECL. The model had no stakes in Pakistan except this case, the counsel said.
He cited the recent Supreme Court judgment which allowed retired Gen Pervez Musharraf to leave the country after his name was removed from ECL. He emphasised that placing a name on the ECL was a policy matter.
It was for the federal government to decide whether or not the model was needed in Pakistan, the counsel said.
Defending the accused, Latif Khosa quoted the 1991 Supreme Court judgment which nullified unfettered authority of the government of not disclosing information regarding prohibition placed on a person from leaving the country even if he or she had valid travel documents and stated that this was against the commandments of God who even had given a notice to Satan before punishing him.
Mr Khosa said the government did not apply its mind while placing his client’s name on the ECL.
Ms Ali’s name figures nowhere on the rules governing the exit from Pakistan since she was neither involved in corruption, misuse of power causing loss to the state and economic crime. Justice Faisal Arab observed that smuggling was an economic crime.
